Changing Name or Beneficiary

Change of Name

1. Employee completes applicable sections on the Change Form indicating the employee's name before the change, effective date of change and only the information that is changing (i.e., new name(s))

2. Submit this form to the insurer

Change of Beneficiary

The insurance laws of the province in which the employee was a resident when the initial beneficiary was appointed govern beneficiary changes. Note that the beneficiary(ies) are declared to be the person(s) named on the most recently dated beneficiary form.

Complete and sign the Change Form form and submit the original copy of the form to the insurer.
